Gastric Balloon Categories
1. Endoscopic Single Balloons
How it is placed: Placed through the mouth under endoscopic guidance with sedation or general anesthesia, then inflated with saline or air.
Duration: Usually 6 months (some models 12 months)
Removal: Again requires endoscopy plus sedation. A second procedure is mandatory.
Well-known examples: Orbera (Apollo Endosurgery), Medsil, BIB
Advantages:
- Placement under endoscopic visual control
- Longer-duration (6–12 months) placement
Disadvantages:
- Two separate endoscopic procedures (placement + removal)
- Risk of sedation/anesthesia twice
- Day procedure or hospital admission
2. Dual Balloons
Definition: Consists of two separate balloons on a single catheter. Theoretical advantage: a more stable position and a lower risk of passing through the gastric outlet.
Placement: Endoscopic, requires sedation
Duration: 6 months
Disadvantages:
- Again endoscopy plus sedation for both placement and removal
- No clear significant efficacy advantage over single-balloon systems
3. Liquid-Filled Endoscopic Balloons vs Air-Filled
Liquid-filled: Saline plus methylene blue dye is used. If the balloon unexpectedly deflates early, this is detected by observing blue-colored urine.
Air-filled: Lighter, with better tolerance in some patients. However, detecting early deflation is more difficult without a visual marker.
4. Allurion Swallowable Balloon
How it is placed: Inside a gelatin capsule, it is swallowed with water along with a thin catheter. No endoscopy or sedation is required.
Fluoroscopy confirmation: Before it is inflated through the catheter, its position is confirmed with fluoroscopy or ultrasound.
Duration: ~16 weeks (4 months)
Removal procedure: None. The balloon deflates on its own and is passed out through the digestive system.

Why Are 16 Weeks Enough?
Allurion's shorter duration may seem like a disadvantage. However, the evidence paints a different picture:
According to a 522-patient study by Caballero et al., 95% of patients maintained their weight loss one year after the 16-week Allurion program (Obes Surg 2025, PMID: 40676353). This shows that it is not the length of the mechanical duration, but the behavioral framework of the program, that is decisive.
The 2017 US FDA Warning: Proper Context
In 2017, the FDA reported death cases linked to certain endoscopic gastric balloons. This warning:
- Primarily targeted endoscopic balloons (Orbera, ReShape)
- Allurion was not within the scope of these warnings at that time
- Following the warning, safety protocols across the industry were strengthened
Allurion's receipt of FDA PMA (Premarket Approval) in February 2026 is an official indication that its safety profile has undergone the highest level of review.

Endoscopic balloons:
- Those who want a longer placement duration (6–12 months)
- Those for whom an endoscopy is already planned
