Are GLP-1 Medications Enough on Their Own?
GLP-1 receptor agonists (GLP-1 RA) have gained an important place in obesity treatment. However, there is a critical problem: weight is regained when the medication is stopped.
In a recent meta-analysis that included 18 randomized controlled trials (n = 3,771 patients), an average regain of +5.63 kg was observed when GLP-1 RA was discontinued (EClinicalMedicine/Lancet, 2025, PMID: 41399474).
This finding shows that relying on a single tool for long-term weight management may not be sufficient.
Why Does the Combination Approach Make Sense?
The protocol that combines the Allurion swallowable gastric balloon with a GLP-1 RA is based on the following mechanisms:
- Volumetric restriction (balloon) + hormonal regulation (GLP-1): Different mechanisms, synergistic effect
- Behavioral reprogramming: The 16 weeks during which the balloon is active is a critical window for establishing new eating habits
- Discontinuation support: When the balloon process is complete, habits have been established, and the GLP-1 RA dose can be reduced
Eli Lilly Collaboration — Preliminary Results
According to the preliminary results of the combination therapy program announced between Allurion Technologies and Eli Lilly in February 2026 (n = 76, press release):
- Low-dose GLP-1/GIP receptor agonist (2.5–5 mg) + Allurion combination
- Average TBWL at the end of 16 weeks: 23%
- This value is significantly higher than that achieved with the balloon alone (14.9%) or with a low-dose GLP-1 RA alone
Important note: These results are preliminary findings and have not yet been published in a peer-reviewed journal. A peer-reviewed publication is expected.
Who Is a Candidate?
The combination protocol is currently in the research phase. In current clinical practice:
- Patients with BMI ≥ 35 and an accompanying comorbidity
- Those who have had insufficient results with a single method
- Those in preparation for bariatric surgery (bridge therapy)
It is recommended that you make the decision together with your specialist physician.
Conclusion
GLP-1 RA and the swallowable gastric balloon are not competitors but complements to each other. The difference in mechanism makes this combination promising. As clinical evidence strengthens, this protocol may become part of standard care.
